Current undergraduate students wishing to enroll in IBEHS 3H03, 3I06 or 4H03 must complete the attached form. Completed forms can be uploaded to the Microsoft Forms Submission Link https://forms.office.com/r/H7eTgrKYBc (also found within the Project Permission Form). The deadline to upload completed forms is five business days before the last day of enrolment (add/drop deadline).

Please use this form to request a course conflict waiver to request permission to enroll into two or more courses that are in conflict. Completed forms can be sent to ibiomed@mcmaster.ca for further processing. Please ensure this form is signed by both course instructors.

Current undergraduate students enrolled in the HESE program who are entering their final year in the Fall term must complete this form and send it to macdonml@mcmaster.ca by June 1.
