B.Eng Students:

B.Tech Students:
• Employer Interim Evaluation - (Word Format)

All Students:
• Work Term Report
• OSAP form - (for 8-16 month work terms only)
• Work Term Contracts (this form needs to be filled out before you start your work term. We will not accept placement information after the work term has been completed).
